Key Responsibilities for Health Center Coordinator:
•    Responsible for two Health Centers
•    Coordination of ancillary services within the IL/ALF and memory support facilities
•    Assist with patient needs within Home Care

Key Requirements Health Center Coordinator:
•    Must be licensed as an LPN or 40 hour CMA in the State of Nebraska
•    Must be self-motivated with the ability to work autonomously
•    Strong organizational skills
•    Previous homecare experience preferred
•    Previous Geriatrics experience preferred

The Health Center Coordinator is responsible to ensure that the health care needs of residents are monitored and necessary interventions are made.  They will coordinate outside health care providers to regularly offer services at eth Grand Lodge as well as maintain a trusting rapport with Grand Lodge residents to be the initial point of contact for medical service needs for all residents.


 


PRIMARY RESPONSIBILITIES: 




  • Symbol"">Scheduling/Coordination of monthly outside medical service.


  • Symbol"">Assessment/Onboarding for new move ins (risk assess/intro to HC).


  • Symbol"">Scheduling/Coordination of health at home visits and associated resident needs.


  • Symbol"">Assessment/Marketing/Coordination of referrals to other Hillcrest Health Services.


  • Symbol"">Updating of Resident Status/Coordination of resident needs with the team.


  • Symbol""> Symbol"">Monitoring/Assessing residents drop-in questions, needs, vitals, etc.

Symbol""> Times New Roman"">KEY REQUREMENTS:




  • Times New Roman"">Completion of Nursing Assistant Certification (CNA) – Currently in good standing on the Nebraska Nurse Aide registry


  • Times New Roman""> Times New Roman"">One year experience in a similar position is preferred.
