·        Graduate of an accredited nursing program and has experience as a Registered Nurse.

·        NOTE:  PRN position must have a minimum of two (2) years of experience as a Registered Nurse

·        Registered Nurse licensed or eligible to practice in Virginia.

·        BSN is preferred; all new Registered Nurses with an Associate's Degree in Nursing will be required to obtain their BSN within five years.

Home Health – April 2017

a. Possess a current, valid driver's license; must meet the driver suitability criteria for MWHC.

b. 1 year recent medical/surgical nursing experienced required.

c. Provides for the physical, mental and social well-being for individuals and families by assessing, planning, implementing and evaluation for nursing care.

d. Performs the comprehensive assessment and develops/implements and evaluates the plan of care as identified on admission and subsequent home visits.

e. Provides appropriate and through education to patients and their caregivers as it relates to the plan of care.

f. When assigned, serves as case manager to coordinate multidisciplinary services per the plan of care.

g. Maintains acceptable productivity standard as determined by agency leadership.

h. Participates in multi-disciplinary team meetings in order to share information and assist with patient management.

i. Participates in staff and administrative meetings.

j. Participates in quality and performance improvement activities.
